Numerous clinically relevant allergenic molecules enhance the performance of specific (s) IgE tests and improve the specificity of allergy diagnosis. This review aimed to summarize our current knowledge of the high-molecular-weight allergens involved in the development of occupational asthma and rhinitis and to critically analyze the contribution of component-resolved diagnosis in the management of these conditions.
There is a lack of standardization and validation for most available extracts of occupational agents, and assessment of sIgE reactivity to occupational allergen components has been poorly investigated, with the notable exception of natural rubber latex (NRL) and wheat flour.

Naturally occurring botulism is rare, but a large number of cases could result from unintentional or intentional contamination of a commercial food. Despeciated, equine-derived, heptavalent botulinum antitoxin (HBAT) is licensed in the United States. Timely treatment reduces morbidity and mortality, but concerns that botulinum antitoxin can induce anaphylaxis exist.

The diagnosis of allergic reactions during anesthesia is difficult. For example, cardio-respiratory symptoms may be due to the accompanying pharmacological effects of anesthetics and poor interpretation of the reaction during anesthesia. It is important to distinguish whether a real allergic reaction has occurred.

Food Allergen Labeling: A Latin American Approach.

J AOAC Int 2018 Jan 5;101(1):14-16. Epub 2017 Dec 5.
Maria Cristina Lopez
Food allergy is a public health concern almost all over the world. Although most of the countries that regulate the declaration of allergens in prepackaged foods include the list recommended by the Codex Alimentarius, some countries have added other allergens to this list due to prevalence data and regional incidence, whereas others have incorporated exceptions for some products derived from allergenic foods. Within this context, the situation in Latin America regarding these regulations is diverse.

Latex allergy confounded by IV infiltration presents a serious problem for pediatric patients. If unrecognized, it can lead to serious neurologic deficits, loss of limb mobility, compartment syndrome, and ultimately lasting disability. Appropriate early recognition can prevent progression to these devastating outcomes.

[Perioperative Anaphylaxia on Drugs].

Anasthesiol Intensivmed Notfallmed Schmerzther 2017 Oct 19;52(10):704-715. Epub 2017 Oct 19.
Wolfgang Pfützner, Hinnerk Wulf
The diagnostic evaluation of perioperative anaphylaxia is of great importance for the medical care of the affected persons in the context of renewed interventions in general anesthesia. The most frequent triggers include muscle relaxants, antibiotics, latex and analgesics of the type of the cyclooxygenase inhibitors, but other perioperatively administered drugs such as opioids, anesthetics, benzodiazepines, dyes, disinfectants and even corticosteroids or drug additives may also be the cause in rare cases. The most important aim is therefore the identification of the responsible drug, for which a near-time allergological diagnosis should be carried out within the first 4 weeks to 6 months after the reaction.

Idiopathic Anaphylaxis.

Curr Treat Options Allergy 2017 3;4(3):312-319. Epub 2017 Jun 3.
Bright I Nwaru, Sangeeta Dhami, Aziz Sheikh
Idiopathic anaphylaxis is a rare life-threatening disorder with symptoms similar to other forms of anaphylaxis. There is lack of a robust evidence base underpinning the treatment of anaphylaxis and even less so for idiopathic anaphylaxis. Much of the evidence therefore comes from relatively small case series and expert opinion.

Latex allergy emerged as an epidemic of anaphylaxis, occupational asthma, and clinical dilemmas in the 1980s. A systematic recognition, investigation, discovery, epidemiology, and prevention strategy followed. International attention and collaborations of investigators, government agencies, manufacturing, and health policy resulted in near elimination of a global epidemic.

Patricia Beierwaltes, Sally Schoessler
Providing a safe environment for students at risk for anaphylaxis from a latex allergy requires care coordination and collaboration of all members of the school community. Strategies for allergy management include educating the school community, identifying potential exposure to latex, preparing to respond in an emergency, and creating a plan for the future. With the student at the center of sound planning, the school can provide a secure and healthy environment.

The exact prevalence of hypersensitivity reactions related to sexual behaviours is not known; however, they heavily impact on the quality of life and of sex life of affected patients. In fact, not only common respiratory and skin allergies, such as asthma, rhinitis, urticaria and atopic dermatitis, but also food and drug allergy have been found to negatively affect the quality of sex life. Allergic diseases impact on the sexual function in both physical and psychological ways, representing one of the main complaints of a considerable proportion of patients.
